| The requirement for the name of the referral laboratory to be included on the report forms Standard G2.4. This standard requires that the name of a referral laboratory be shown on a report form when results are issued by the referring department. There have been objections raised to this clause but it has been included by the Standards Revision Committee as it is an intrinsic part of the European Standards. It has been agreed by CPA that reports can be issued on behalf of a group on the basis that all the laboratories within that group are accredited and under a single management structure. If any of the group members are not accredited then the reports should state the identity of the laboratory where samples were referred.
